Once we’ve received your call/referral
- SCHHS’s Registered Nurse Supervisor will contact you to schedule an intake assessment, at no charge.
- The nurse will conduct an evaluation and assessment and obtain your medical history, including current medications and treatments in progress. (assessments may take up to 2 hours long)
- Explain the different care options and cost of services
- She will develop your Individual Service Plan, prepared in cooperation with you and your family.
- Then, her responsibility is to match you up with the best possible caregiver. Nurses/Aides are carefully selected depending on the needs, health and preferences of our clients.
- Once the assignment has been arranged, your scheduled service visits will begin.
